 今や産学でゲノム情報の医学応用が一大ブームとなり、人類と疾病との闘い
に新たな歴史を刻むことが期待されている。

 病因遺伝子が発見されれば、特効薬の開発につながり、人類はあまねくその
恩恵を受けられるとの期待が大きい。しかし、それには莫大な資本と高度な技
術集積が不可欠であり、その確保が可能なのは事実上私企業である製薬会社に
ほぼ限られる。たとえばある製薬会社が心筋梗塞の真の病因遺伝子を発見し、
その特効薬の開発に成功したとしよう。その会社は特効薬を大々的に売り出す
であろうが、果たして病因遺伝子の検出試薬を作るだろうか。

 現在心筋梗塞の予防のために投薬治療を受けている患者を放置したとして
も、実際に発症するのは数パーセントである。もし原因遺伝子が分かってしま
えば、恐らく本当に予防が必要な患者は激減し、肝心の特効薬の売り上げは大
打撃を受ける。私企業は収益拡大を最優先にすることを余儀なくされるので、
敢えて損をするような商品をわざわざ作るとは考えられない。

 実際には遺伝子の発現率や環境因子などが影響するので、このように単純で
はないが、同じ状況は病因遺伝子に起因するあらゆる疾患に起こり得る。あま
り注目されていないが、ゲノム創薬に関わる検査試薬の開発が、病因遺伝子の
検出ではなく、副作用を起こす遺伝子の検出に限られている本当の理由はここ
にある。

 製薬会社が特許としてその遺伝子を出願すれば、内容が公開されやがて有効
期限が切れるので、その後は病因遺伝子を持つ人だけが必要な予防措置を受け
る時代が来るであろう。しかし、特効薬だけを特許化し、病因遺伝子について
は秘密にしてしまう可能性も十分に考えられる。既に特効薬が売られているの
に、敢えて莫大な投資をして同じ病因遺伝子を発見しようとする行為は、経済
合理性から見て起こり得ないからである。

 このような事態は人類にとって不幸であるが、私企業の倫理観に任せておい
て解決することはあり得ない。学界に身をおく我々にこそ、そのような事態を
予見し、私企業の利益と社会の利益がうまく釣り合うようなしくみを整備する
よう、リーダシップを発揮することが求められているのではないだろうか。

論 壇 :次世代感染症診療への展望

 感染症診療は遺伝子検査による革新が期待されているが、話題になっている
のは病原体を対象とした遺伝子検査に限られている。確かに診断の迅速化、高
感度化、高精度化あるいは遺伝子レベルの耐性検出など、画期的なメリットは
あるが、さらに発想を転換し、より大きな変革を目指すべきである。

 文献的根拠は定かではないが、臨床経験からは、特定の病原体が感染しやす
い患者が存在し、しかもある患者が感染症を起こしやすい病原体は限られてい
る。特に常在菌による感染症の頻度が増えてからは、その傾向が顕著である。
したがって、常在菌や環境菌を含め、各々の微生物に対する免疫力には大きな
個人差があり、遺伝子がそれを規定する重要な因子であることはほぼ間違いな
い。遺伝的に特定の微生物に対する防御能が劣っている人が、何らかの原因で
局所あるいは全身の免疫機能の活性が低下した状況に陥ると、その微生物が感
染を起こすというシナリオは十分考えられる。

 そこで、もし各個人がどの微生物に弱いか(易感染体質)を予め遺伝子で調
べてそれを補ったり(免疫補完療法)、免疫機能の活性低下を検出してそれを
是正すること(免疫回復療法)ができれば、感染症診療の方法論は一変する。

○発症後治療
 現在:感染症発症→病原体の推定・病原体の検出開始→治療開始(抗生物
    質)→治療経過と検査結果の確認→治療方針の再検討・病原体の再検
    出(→治療経過と検査結果の確認に戻る)
 将来:感染症発症→病原体の検出・易感染体質と免疫機能の検査→テーラー
    メード治療開始(抗生物質・免疫補完療法・免疫回復療法)→治療経
    過の確認→治療方針の再検討・病原体の再検出(→治療経過の確認に
    戻る)

○予防的治療
 現在:免疫力低下を招く病態(手術侵襲・発癌・移植・エイズなど)→感染
    し易い病原体の推定→予防治療(抗生物質)→実際に発症→上記の発
    症後治療へ
 将来:免疫力低下を招く病態(手術侵襲・発癌・移植・エイズなど)→感染
    し易い病原体の推定・易感染体質と免疫力の検査→テーラーメード予
    防治療(無処置または免疫補完療法・免疫回復療法)→実際に発症→
    上記の発症後治療へ

○アウトブレイク防止
 現在:特異抗体あるいは特異的細胞性免疫の測定→ワクチン接種・感染経路
    の遮断
 将来:予め易感染体質と免疫機能を把握→流行時のみテーラーメード予防治
    療(無処置または免疫補完療法・免疫回復療法)・感染経路の遮断

 このような視点からの研究開発は、産学を通じてもまだあまり見受けないよ
うに思うが、私の勉強不足のせいであろうか。もしそうでなければ、是非同士
を募って自ら取り組みたいものである。

Opinion: A Pitfall in Expectations for Molecular Diagnostic Tests

There is a great boom on the human genome in both the academic world
and the industrial world, and a new page will begin in human history
of battle against disease.

If a key gene responsible for a disease will be found, it is expected
as a matter of course that an ideal medicine for it will be developed
and all mankind will receive its benefit. But development of a
medicine requires a huge investment and accumulation of most advanced
technologies, and virtually only drug companies can afford to provide
them. Suppose that a drug company found a key gene of cardiac
infarction and succeeded in producing an ideal medicine. This company
will sell the medicine with extensive sales promotion, but will it
make a diagnostic test for detecting the key gene?

Only a few percent of patients who continue to take preventive
medicine against cardiac infarction will develop the disease even if
all of them ceased the treatment. If the key gene will be detected in
advance, the number of patients who need prevention will sharply
decrease and sales of the ideal medicine will be severely damaged. A
profit company is obliged to pursue profit and it will by no means
make a product that will cause a huge loss.

Actually a gene expression rate and environmental factors should be
taken into account and things are not so simple like this. But a
similar situation will emerge from every disease. It should not be
overlooked that this is the true reason why development of molecular
diagnostic tests related to medication is focused not on detection of
key genes of diseases, but on genes causing side effects.

Provided that a company protects a patent of a key gene, it will be
open to the public and sometime will be used for finding people who
exactly need prevention. But it will protect a patent of only a
medicine derived from the gene and will conceal it from the public,
because no one will try to make an investment for finding the gene
after the medicine goes on the market.

Although abovementioned situations are unhappy for mankind, there will
not be any solution only by relying morals of profit companies. We who
live in the academic world must foresee such situations and must show
good leadership to balance private profit with public welfare.

Opinion: A New Generation Strategy of Controlling Infectious Diseases

Molecular diagnostic tests will have a great impact on control of
infectious disease and interest seems to be focused on the tests
detecting nucleic acid unique to each pathogen. However, we should
consider another approach aiming at a larger breakthrough.

It has been known from clinical experience that there are people who
are easy to be infected with specific pathogens and pathogens that a
patient is easy to be infected with are almost fixed. These phenomena
become prominent since opportunistic infection increased. Therefore,
it is assumed that there large differences in immunity against each
pathogen among individuals and these differences are mainly regulated
by genes. A scenario that an individual who has a gene associated with
less immunity against a pathogen will get infected with the pathogen
when he/she loses intact activities of his/her immune system is
considered reasonable.

If pathogens, against which an individual has less immunity, will be
known in advance (prediction of risky pathogens for a individual),
individual lack of immunity against specific pathogens will be
compensated for by a medical intervention (immune complement) and
decreased activities of individual immune system will be assessed and
recovered by a medical intervention (immune recovery), then there will
be complete changes in both control and treatment of infectious
disease.

Treatment after Development of Symptoms
-at present:
  development of symptoms
 ->estimation and detection of pathogens
 ->antibiotic treatment
 ->following-up a clinical course and collecting laboratory data of
   pathogens
 ->review of treatment and pathogens
 -> (go back by two lines)
-in future:
  development of symptoms
 ->detection of pathogens, prediction of risky pathogens for the
   individual and assessment of immune activity of the individual
 ->tailor-made treatment (antibiotic treatment, immune complement and
   immune recovery)
 ->following-up a clinical course
 ->review of treatment and pathogens
 -> (go back by two lines)

Preventive Treatment
-at present:
  clinical situations that damage immune activities (surgery,
  malignancy, transplantation, AIDS, etc.)
 ->estimation of possible pathogens
 ->preventive treatment (antibiotics)
 ->(if symptoms will develop, go to 'Treatment after Development of
   Symptoms'
-in future:
  clinical situations that damage immune activities (surgery,
   malignancy, transplantation, AIDS, etc.)
 ->estimation of possible pathogens, prediction of risky pathogens for
   the individual and assessment of immune activity of the individual
 ->tailor-made prevention (following-up only, immune complement and
   immune recovery)
 ->(if symptoms will develop, go to 'Treatment after Development of
   Symptoms'

Prevention of an Outbreak
-at present:
  detection of pathogen-specific antibodies and immune cells
 ->vaccination and breaking the infection route
-in future:
  prediction of risky pathogens for the individual and assessment of
  immune activity of the individual in advance
 ->tailor-made epidemic prevention (following-up only, immune
   complement and immune recovery), breaking the infection route

If these approaches are new to both academic and industrial world, I
would like to challenge to them together with like-minded people.
