特別寄稿:マイクロアレイ技術の臨床微生物検査への応用の実際とその将来像

 DNAマイクロアレイとはガラス等の基板上に多数の反応サイトを並べたもの
で、個々のサイト上でDNAまたはRNAをハイブリダイズさせ目的とするターゲッ
ト遺伝子を検出する手法である。

 この手法は医療の分野ではヒトの病態に関わる特定遺伝子の検出や、患者か
ら採取した試料からの病原微生物の遺伝子の検出等に応用できると言われてい
る。本法は1990年代前半から米国を中心に注目を浴び、1998年のSCIENCEで
NEWS FOCUSとして詳細が取り上げられている。ここでは微生物検査へ応用と敗
(菌)血症の迅速診断としての利用価値について述べる。

 敗(菌)血症の診断におけるGold standardは、患者からの血液を検体とした
培養検査である。通常、血液の培養には細菌類が発育し易いbrothが入った培
養ボトルを用い、採取した血液を接種し、自動培養装置にて5日間連続監視培
養するのが一般的である。しかし、培養後陽性と判定された場合、培養ボトル
より内溶液を採取し、常法の分離培養、同定試験を経る事から、起炎菌の確定
までには陽性判定後1〜2日を要する。これに対し、我々が考案したDNAマイク
ロアレイによる敗(菌)血症の診断は、検体を採取後24時間以内で起炎菌を確定
する事を目的とした。

 DNAマイクロアレイにスタンピングした菌種は日常の血液培養検査より分離
頻度の高い菌種を中心とした各菌種の基準株57種を用いた。DNAマイクロアレ
イの作成は各基準株からDNAを抽出し、PCRによって16S rDNA 5'領域500bpの増
幅を行った。また、50merの3つの可変領域のオリゴヌクレオチドを合成し、計
4つのDNAをプローブとした。このprobe DNAをアレイヤー(日立ソウフトウエア
エンジニアリング)によってアレイ(住友ベイクライト)にスタンピング(埋め込
み)を行った。

 ターゲットDNAの調整は患者血液を接種したBactec 96FボトルをBactec9240
systemにて8〜18時間培養後、ボトルよりsampleを1mL採取し、溶血処理後赤血
球成分を除去した。

 DNAを抽出、精製しPCRにて16S rDNA 5'領域 500bpの増幅を行い、cy5で蛍光
標識した。反応と解析はProbe DNAと標識したターゲットDNAによってハイブリ
ダイゼーションを行い、スキャナー(Bio Medical Equipment)によって読み
とった蛍光シグナルをDNASIS Array(日立ソフトエンジニアリング)によって解
析し、菌種名を決定した。

 敗(菌)血症が疑われた患者245例におけるDNAマイクロアレイ法および従来法
である培養法で比較検討した結果をFig.1に示した。245試料中培養法にて陽性
となった例は56試料で、これらは全例DNAマイクロアレイで陽性を示した。こ
れらの陽性例のうち両法とも検出菌が完全に一致したのは43試料で、残り13試
料は培養法によって検出された菌種以外に異なる菌が検出された。培養法陰性
でDNAマイクロアレイ陽性例は16例であった。逆に培養法陽性、DNAマイクロア
レイ陰性例は全くなかった。

 過去に我々は検体採取後直ちにDNAマイクロアレイ法によって検査を実施し
た結果、採取後初期の検体には多数のDNAが含まれ、これらによって培養法と
は相関しない成績となってしまう事を報告した。今回は採取後の培養ボトルを
pre-incubationする事により起炎菌をある程度増菌させ、不要なDNAと選択的
に区別する事に成功し、培養結果と相関する成績が得られた。また、培養では
検出不可能な菌種も検出する事が可能となった。

 敗(菌)血症の治療上重要なことは、侵入門戸と起炎菌の迅速な決定であるこ
とには変わりがない。本稿で述べたDNAマイクロアレイによる敗(菌)血症起炎
菌の検査は、その迅速性によって敗(菌)血症の早期治療に十分貢献できるもの
と思われる。しかしながら、本手法はチップ上に展開された菌種のみの簡易同
定である事から、薬剤感受性やその後の疫学調査については未解決である。
従って、現段階では、従来の細菌学的検査と併せて利用することで医療的価値
が生じるものと思われる。

 反面、簡便かつ身近になった遺伝子検査の万能崇拝には、将来に危惧をも感
じる。病原微生物の実態を遺伝子レベルだけではなくマクロで捕らえる事も欠
かせない問題である。現に病原体そのものを無視し遺伝子の検出だけに頼る検
査は疫学の解析に大きなリスクを背負う。

 将来の感染症学の進歩へバランスのとれた検査法の向上と治療に直結した迅
速検査およびそれらを取扱う検査員の育成を望む。

Identification of bacteria by blood culture requires one or two days
after positive reaction is observed. A new diagnostic method developed
by us using microarray identifies 57 bacteria in a blood specimen
within only 24 hours after sampling.

Target DNAs are extracted from 1ml of culture fluid of Bactec 96 F
bottles incubated for 8-18 hours after inoculation. After
purification, 500 base pairs at a 5' region of 16S rDNA are amplified
by a PCR method and labelled with cy5. DNA probes selected by us are
whole this 500 base pairs and 50mer of three variable sub-regions of
it. Fluorescent signals of these PCR products are captured by a
scanner and analyzed using DNASIS Array software.

Out of 245 blood specimens collected from septic patients, 56
specimens showed positive reaction by culture and all of them showed
positive results also by our method. Sixteen specimens of the rest
showed positive results only by our method. Concerning the 56
specimens that showed positive reaction by culture, our method
identified not only the same bacteria that blood culture did, but
also other bacteria in 13 specimens.

During development of our method, much more kinds of bacteria were
identified by our method than by culture from the same sample when
analysis was done immediately after sampling. We inserted a pre-
incubation process before analysis to select important bacteria from
trivial ones aiming at similar results are gotten by our method and by
culture.

Although our method will have a significant advantage in reduction of
time consumed before diagnosis, it has also a certain limitation and
other current laboratory tests should be wisely used together.

We would like to have well-balanced progress of laboratory tests,
quicker diagnostic methods that have direct effects in cure and
laboratory technologists who use them efficiently.
