 癌とは多段階の過程を経て成立するが、その過程内で遺伝子の変容が蓄積し
ていく。ここ数十年の間癌研究では、ゲノム情報に基づき遺伝子変異の検出に
多大なる努力が払われてきた。その結果に関与している種々の遺伝子が同定さ
れた事も事実である。しかしながら遺伝子の変異だけでは必ずしも発癌のメカ
ニズムを説明できない事が最近わかってきた。

 エピジェネティックスとはDNAの塩基配列の変化を伴わず、代々継承されて
いく遺伝子発現の変化を研究する新しい学問領域である。これにはDNAメチ
レーションやヒストンのアセチル化、脱アセチル化といった現象が主に含まれ
ている。現在エピジェネティックの変化は、腫瘍化に関与する新たなメカニズ
ムとしてとらえられている。

 遺伝子のプロモーター領域におこるDNAメチレーションは、その遺伝子の発
現を抑制する。これには現在のところ二つのメカニズムが関与すると考えられ
ている。ひとつは、転写因子がメチル化されたDNA領域に直接結合できなくな
るというもの、もうひとつはMECP2などのメチル化DNA結合タンパクを介して遺
伝子発現の抑制がおこるというものである。メチル化DNA結合タンパクはヒス
トン脱アセチル化酵素(HDACs)をはじめとする遺伝子発現抑制に働くタンパク
群を複合させる。HDACsはヒストンH3 やH4のリジン残基に反応し、そのアセチ
ル基を取り除く。脱アセチル化されたヒストンは陽電荷を帯び、その結果クロ
マチンは凝縮、転写因子の遺伝子プロモーター領域へのアクセスが制限される
というものである。

 腫瘍細胞では、hypermethylation、すなわち正常ではメチル化されていない
領域のメチル化の程度が増す現象がVHL やp16といった癌抑制遺伝子のプロ
モーター領域で認められる。逆にHypomethylation、正常ではメチル化されて
いる領域の脱メチル化現象もヒトの腫瘍で報告されている。例えば1番や16
番染色体のpericentric heterochromatin領域は正常では強くメチル化されて
いるが、腫瘍ではメチル化されていない事があるとわかった。非常に興味深い
ことに、Immunodeficiency, Centromeric region instability, Facial
anomalies syndrome (ICF)なる疾患においても同領域は脱メチル化されてお
り、加えてde novo methyltransferase であるDNMT3bに変異が見つかった。繰
り返し配列に起こるこれらの脱メチル化現象はゲノムの不安定性を引き起こす
と考えられている。

 今後、エピジェネティクな変化の研究は癌の診断のみならず予後や治療効果
の推定に役立つと考えられる。

 この度遺伝医学関連10学会が共同で「遺伝学的検査に関するガイドライン」
を発表した。遺伝子検査については、研究目的の場合については既に日本政府
からガイドラインが発表されているが、臨床適用については初であり、注目さ
れる。

Cancer is thought to be caused through a multistep process during
which aberrations of various genes accumulate. For decades, vast
efforts have focused on the detection of genetic mutation based on the
genomic information that resulted in the identification of a lot of
genes responsible for tumorigenesis. Recently, it is, however, clear
that only genetic alteration will not necessarily explain the
mechanism of tumorigenesis.

Epigenetics is a young field investigating heritable changes in gene
expression which occur without a change in DNA sequence. It mainly
includes DNA methylation and modification of histone due to
acetylation or deacetylation. Epigenetic changes are now recognized as
additional mechanisms contributing to the malignant phenotype.

DNA methylation in the promoter regions of genes is correlated with
gene silencing. Two underlying mechanism have been proposed until now.
First, transcriptional factors are directly inhibited to bind at the
methylated site. The second mechanism involves methyl-binding proteins
such as MECP2. These proteins recruit repressor complexes including
histone deacetylases(HDACs), which remove acetyl groups from lysine
residues of histone H3 and H4. The positive charged-deacetylated
histones cause condensation of chromatin and limit access of
transcriptional factors to promoter regions of genes.

In tumor cells, hypermethylation,the gain of methylation at sites
usually unmethylated, is found in the promoter lesion of tumor
suppressor genes such as VHL and p16. Hypomethylation, the loss of
methylation at normally methylated sequences, has also been reported
in human malignancy. For example, pericentric heterochromatin regions
on chromosomes 1 and 16, which are heavily methylated in normal cells,
are found to be demethylated in tumor genoms. Very interestingly,
these regions are also hypomethylated in patients with
immunodeficiency, centromeric region instability, facial anomalies
syndrome (ICF) in which a mutation of the de novo methyltransferase
DNMT3b has been detected. These demethylation events occurred in
repetitive elements are thought to cause genomic instability.

From now on, investigation of epigenetic changes in cancer will useful
in predicting prognosis and treatment benefits as well as in diagnosis
of cancer.

Ten academic societies related to genetic medicine in Japan issued a
clinical guideline for molecular diagnostic testing in unanimous
cooperation. Japanese government has issued already a research
guideline for molecular testing, but it is worthy of notice that this
is the first one for its clinical application.
